• Must provide documentation of being assigned to serve
• Must provide an evaluation of performance
• Must submit verification form
• Supervision must be for a placement period of not less than 300 clock hours as determined by the sponsoring college/university

• Candidate must be an active member
• Candidate must participate in 75% of meetings (sign in and out), but not less than six meetings in a school year
• Must submit a verification form
• Three SB-CEUs awarded for participation for a full school year

Documentation must include original attendance sheet and agenda.  Email documentation is not allowed.  Activity must take place between September to June (current school year) only.  Participants can earn three SB-CEUs with a maximum of nine within a five-year period.
